Forms and Flyers (revised May 2009)
Below you will find a variety of Categories and forms you are required to fill out for the 2009-2010 TOHS BAND SCHOOL YEAR. for forms that are used for our Band. Please go through all the forms and be sure to complete all that apply to your student. All forms are fillable (using Adobe Acrobat Reader) and we ask that you complete the forms, and bring them with you to the band meeting on May 26, 2009. All students are required to have these forms for the 2009-2010 school year.
CVUSD FORMS:
These forms must be filled out for ALL TOHS BAND Members.

- Field Trip Authorization and Medical Authorization Form
- TOHS Band Authorization for Treatment and Permission to Participate Form
- CVUSD Co-Curricular Code and Conduct
TOHS Band Permission for Absence
One-week prior notice is required for any planned absence as per School District policy. The teamwork required of Band members makes prior notification vital. All performances and rehearsals are mandatory. If a planned absence is unavoidable, complete this form and return it to the band office at least one week before a rehearsal absence and two weeks before a performance absence.
TOHS SCHOOL EQUIPMENT USAGE FORMS:
These forms are used by all Color Guard, Percussion, and Instrumental Students using CVUSD School Equipment. Please be sure to attach payment with form.

ALL members of the TOHS Band who play a musical instrument must fill-out the following form:
TOHS COLOR GUARD ASSESORIES FORM:
This form is used by all Color Guard members. Please be sure to attach payment with form. Remit check to: TOHS BAND BOOSTERS
TOHS REQUIRED UNIFORM AND ALTERNATE CLOTHING FORM:
This form is used by all members ordering Band Uniform clothing and mandatory alternate clothing. The Alternate Clothing Order form can also for any additional items and booster wears: Please be sure to attach payment to the form.

TOHS UNIFORM CARE AND AGREEMNT FORM:
This form must be filled out for ALL TOHS Band Members
TOHS UNIFORM HEMMING FORM:
This form is used by any parent authorizing and paying for their students uniform to be hemmed by the Boosters. Please be sure to attach payment with form. Remit check to TOHS BAND BOOSTERS
ISSA Funds Transfer Request
To transfer funds from a student's ISSA fund to pay for an event or other band approved expenditure, the following form must be used:
